Transitions Partner

BHITC provides Open Table as an after-care program to agencies serving at-risk populations, including homeless, youth aging out of foster care, working poor, community re-entry and under-fathered youth.  Open Table places these individuals among small groups of trained BHITC volunteers, who offer encouragement, life skills training, mentoring and more, for up to one year.

Although new to Northeast Ohio, Open Table has a demonstrated track record of working in partnership with social service agencies, government and faith communities across the country to create an effective model that helps extend case management services for clients as they transition away from in-house or institutional settings.
